Integration with NUMBAS

I would like to embed a worksheet in NUMBAS following this recording but I can’t make it work as things changed. Could anyone help me with that please?

I would like to set up an activity in Geogebra, that students have parametric equations, but have to draw the curve using Cartesian equation. I couldn't set up a worksheet in Geogebra so I changed the question. But I still would like to learn how to set up a worksheet that can be used in NUMBAS. This is the question that I am not happy about:

Just to add it links to Geogebra so each student has different parameters a,b,c and d and the graph changes accordingly.
